For a Remote client for please fill out and return with an optional photo for each family member having a session. Remote Client Form
For an office visit complete and bring this AJ intake form 2011. The form explains some procedures and a bit of what to expect during a hands-on session. There will be additional forms for those of you who want to work on your diet or exercise. Your therapist will review your information the during the first visit and you will be able to ask questions.
